Approved drugs come first in each group. This summarizes each compound’s own page; it isn’t medical advice, and a doctor or pharmacist can answer for a specific person.

From a product label 18

CerebrolysinApproved drug
LabelNeither label gives liver advice. The EU label notes its safety wasn’t studied in people with other severe body-wide diseases.
ExenatideApproved drug
LabelNot studied in liver disease. Because the kidneys clear exenatide, the label doesn’t expect liver problems to change its blood levels.
GHRP-2Approved drug
LabelNot addressed in the label, which lists ALT rises (frequency not known) and says the drug likely leaves mainly through bile. Four people with liver impairment had no reactions after the test.
hCGApproved drug
LabelThe labels give no liver-specific advice or dose change. In women, severe ovarian overstimulation has caused temporary abnormal liver tests.
LiraglutideApproved drug
LabelExperience in mild to severe liver impairment is limited, so both labels advise caution; Victoza’s calls for no dose change. In a small trial in NASH, a fatty liver disease, it resolved more often than on placebo.
Melanotan 1Approved drug
LabelRuled out by the EU and Canadian labels in liver impairment or significant liver disease; the US label says the effect of liver impairment is unknown.
OrforglipronApproved drug
LabelNo dose change for mild or moderate liver impairment; not recommended in severe impairment (Child-Pugh class C, the most advanced stage).
OxytocinApproved drug
LabelNot studied in liver impairment. The UK label says big changes in clearance are unlikely, since enzymes outside the liver also break it down.
PT-141Approved drug
LabelNo dose change for mild to moderate impairment (levels up 1.2- to 1.7-fold). Severe impairment wasn’t studied, and the label advises caution. One case of acute hepatitis was reported in trials.
SemaglutideApproved drug
LabelNo dose change: mild to severe liver impairment didn’t alter drug levels. Wegovy is approved for MASH (fatty liver disease with inflammation) with moderate to advanced scarring, but not for cirrhosis.
SS-31Approved drug
LabelNo dose change. Lab tests found no breakdown by the liver, so the label expects liver impairment not to alter drug levels; no liver-impairment study is described.
TesamorelinApproved drug
LabelThe label says how the body handles it in liver impairment hasn’t been established. In people with HIV and fatty liver, a 12-month trial found less liver fat than on placebo.
ThymalinApproved drug
LabelThe Russian label says there are no data on dosing in liver impairment.
Thymosin Alpha-1Approved drug
LabelApproved in countries such as Indonesia for hepatitis B with compensated liver disease (no signs of liver failure). Liver enzymes can flare during treatment; the label says to continue unless liver failure signs appear.
TirzepatideApproved drug
LabelNo dose change: mild to severe liver impairment didn’t alter drug levels. In a phase 2 trial in MASH, a fatty liver disease with scarring, it improved liver inflammation; it isn’t approved for that.
TriptorelinApproved drug
LabelExposure was 3.7 times higher in men with liver disease, and the labels give no dose change. Fatty liver disease, including cirrhosis, has been reported with GnRH agonists.
VIPApproved drug
LabelInvicorp’s label says no formal studies have been done in liver impairment. The NIH’s IV trial in COVID-19 excluded people with end-stage liver disease.
SermorelinHuman studies
LabelNo data, the former US label says; it wasn’t studied in people with liver impairment.

From human studies 10

GlutathioneApproved drug
TrialThe label gives no liver dose advice. Glutathione has been given by vein for liver disease, and in a small oral study in fatty liver, ALT fell.
L-CarnitineApproved drug
TrialThe label gives no advice for liver disease. In supplement trials it lowered liver enzymes, most in people with liver disease, and a trial in critically ill liver patients is recruiting.
ARA-290Human studies
TrialNot studied: trials excluded significant liver disease, and no data exist on how liver impairment changes drug levels.
CagrilintideHuman studies
TrialIn a single-dose study of 32 people, mild to severe liver impairment didn’t change cagrilintide levels, safety or tolerability. Longer use in liver disease hasn’t been reported.
LarazotideHuman studies
TrialNot studied in liver disease; the long COVID trial excluded liver enzymes over 3 times normal. It acts in the gut and wasn’t detectable in blood.
MK-677Human studies
TrialNot studied in liver disease. Liver enzyme rises occurred in several trials, and in 12 adults with fatty liver, 6 months didn’t reduce liver fat.
RetatrutideHuman studies
TrialA drug-level study in liver impairment finished in 2025, unpublished. In adults with fatty liver, liver fat fell sharply in a phase 2 substudy.
SurvodutideHuman studies
TrialAfter one dose, drug levels were similar in mild to severe cirrhosis and in healthy people, and the researchers judged no dose change necessary; 28 weeks of treatment in mild to moderate cirrhosis was generally tolerated.
Thymosin Beta-4Human studies
TrialNot studied: the ulcer gel trials excluded significant liver disease, and no data exist on how liver impairment changes drug levels.
MOTS-cPreclinical
TrialNot studied for MOTS-c itself. Its modified version CB4211 was given daily for 4 weeks to 11 adults with fatty liver, with no serious side effects, per the company’s report.

A precaution, not studied 40

DSIPApproved drug
PrecautionNot studied in liver disease, and the Deltaran label gives no advice.
GonadorelinApproved drug
PrecautionNot studied in liver disease; the UK label gives no guidance.
MazdutideApproved drug
PrecautionNot studied in people with liver impairment such as cirrhosis, and no drug-level study is registered. Trials in fatty liver disease without cirrhosis are under way.
SelankApproved drug
PrecautionNot studied in liver disease, and the Russian label gives no advice or liver cautions.
SemaxApproved drug
PrecautionNot studied in liver disease, and the Russian labels give no advice.
AOD9604Human studies
PrecautionNot studied in people with liver disease. FDA flagged dose-related liver cell changes in monkeys given it by mouth for 9 months.
ArgirelineHuman studies
PrecautionNot studied in people with liver disease. Lab tests suggest skin use puts little or none into the body.
CJC-1295 with DACHuman studies
PrecautionNot studied in people with liver disease; the trials enrolled only healthy adults.
DihexaHuman studies
PrecautionNo data. The prodrug’s Alzheimer’s trial excluded people with liver impairment.
GHK-CuHuman studies
PrecautionNot studied in people with liver disease. The liver clears excess copper into bile, so liver disease or Wilson’s disease could let copper from repeated injections build up.
GHRP-6Human studies
PrecautionNot studied in people with liver disease.
HexarelinHuman studies
PrecautionNot studied in people with liver disease.
IpamorelinHuman studies
PrecautionNot studied in people with liver disease.
Kisspeptin-10Human studies
PrecautionNot studied in people with liver disease.
LL-37Human studies
PrecautionNot studied in people with liver disease, and how the body breaks down injected LL-37 has never been measured.
MatrixylHuman studies
PrecautionNot studied in people with liver disease; in lab tests on mouse skin, none passed fully through.
Melanotan 2Human studies
PrecautionNot studied. Bremelanotide levels rose 1.7-fold in moderate liver impairment, and its label advises caution in severe impairment.
NAD+Human studies
PrecautionNot studied. The one IV study excluded people with liver failure; in its 8 healthy men, liver tests barely changed.
SNAP-8Human studies
PrecautionNot studied in people with liver disease, and no study has measured how much passes through skin.
TesofensineHuman studies
PrecautionNot studied in liver disease; trials excluded raised liver enzymes. The liver enzyme CYP3A4 clears it, and blocking that enzyme raised exposure by up to 63% in a modeling study.
5-Amino-1MQPreclinical
PrecautionNot studied in people with liver disease. Mouse liver cells broke it down within minutes, so liver disease could change exposure; untested.
AdamaxPreclinical
PrecautionNo data; Adamax has never been studied.
AHK-CuPreclinical
PrecautionNot studied in people with liver disease. It carries copper, which the liver clears into bile, so Wilson’s disease and other copper disorders are the main concern.
BPC-157Preclinical
PrecautionNot studied in people with liver disease. FDA flagged possible liver signals in a 28-day rat test, though other rat studies report liver protection.
CartalaxPreclinical
PrecautionNot studied in people with liver disease; how the body breaks it down is unknown.
CJC-1295 (no DAC)Preclinical
PrecautionNot studied in people with liver disease.
EpithalonPreclinical
PrecautionNot studied in liver disease. FDA found no data on how the body absorbs, breaks down or clears Epithalon.
Follistatin-344Preclinical
PrecautionNot studied in people with liver disease. In rats most of an intravenous dose went to the liver, where follistatin altered regeneration after part of the liver was removed.
FOXO4-DRIPreclinical
PrecautionNot studied in liver disease. How the body breaks it down and clears it hasn’t been measured in any species.
HGH Fragment 176-191Preclinical
PrecautionNot studied in anyone, with or without liver disease.
HumaninPreclinical
PrecautionNo human data. In rodents, humanin and HNG acted through the brain to change the liver’s insulin response and fat output; what they do in liver disease is unknown.
IGF-1 DESPreclinical
PrecautionNot studied in people. IGF-1 is broken down by both liver and kidney, per the mecasermin label.
IGF-1 LR3Preclinical
PrecautionNot studied in people. IGF-1 is broken down by both liver and kidney, per the mecasermin label.
KPVPreclinical
PrecautionNot studied in people with liver disease, and FDA found no measurements of what happens to it in the body.
MGFPreclinical
PrecautionNot studied in people with liver disease, and how the body breaks it down has never been measured.
PE-22-28Preclinical
PrecautionNo human data; how the body breaks it down is unknown.
PEG-MGFPreclinical
PrecautionNot studied in people with liver disease, and how the body breaks it down has never been measured.
PinealonPreclinical
PrecautionNot studied in people with liver disease; how the body breaks it down is unknown.
SLU-PP-332Preclinical
PrecautionNot studied in people with liver disease; how the human body breaks it down is unknown.
TB-500Preclinical
PrecautionNot studied in people with liver disease. Liver enzymes break it down in lab tests, and FDA found no toxicity studies.
