The short answer
Mixed with 1 mL of bacteriostatic water, a 10 mg vial of Survodutide holds 10 mg/mL. A 3.6 mg dose is then 36 units (0.36 mL) on a U-100 insulin syringe, and the vial holds about 2 doses of that size. Taken 1× per week, it lasts about 2.8 weeks.
How much water
The water only sets the strength; the dose and the number of doses in the vial stay the same. Draws are on a U-100 insulin syringe, where 1 unit is 0.01 mL.
| Water added | Strength | Per syringe unit | Draw for 3.6 mg |
|---|---|---|---|
| 1 mL | 10 mg/mL | 100 mcg | 36 units (0.36 mL) |
| 2 mL | 5 mg/mL | 50 mcg | 72 units (0.72 mL) |
| 3 mL | 3.333 mg/mL | 33.3 mcg | 1.08 mL |

Draws over 100 units show in mL. Faded values are under 2 units, which is hard to measure. Check that your vial holds the larger volumes.
Where the doses come from
- Trial Phase 3 maintenance doses of 3.6 or 6.0 mg once weekly, reached by stepping up from a low dose, with pauses or slower steps allowed for stomach side effects. At 76 weeks weight fell 12.2% and 13.0% vs 5.4% on placebo, counting everyone who started.
- Trial Phase 2 tested 0.6, 2.4, 3.6 and 4.8 mg weekly: 20 weeks of step-ups every 2 weeks, then 26 weeks at the assigned dose. Mean loss was 14.9% at 4.8 mg vs 2.8% on placebo.
- Trial MASH (fatty liver with inflammation) phase 2: 2.4, 4.8 or 6.0 mg weekly for 48 weeks, including a 24-week fast step-up. MASH improved without worse scarring in 47%, 62% and 43% vs 14% on placebo.
Doses are what labels, trials or community reports reference, not recommendations. Survodutide’s page has the cycle, side effects, cautions and sources.
Mixing and storage
- PrepareLet a refrigerated vial come to room temperature, and wipe both stoppers with an alcohol swab.
- Add 1 mLDraw 1 mL of bacteriostatic water and let it run slowly down the inside wall of the vial.
- Swirl, don’t shakeSwirl gently until the powder dissolves and the liquid is clear.
- Label itWrite the date and the strength, 10 mg/mL, on the vial.
Storage: No label exists. Usual practice: powder frozen or refrigerated; mixed vials at 2–8 °C, protected from light.
The reconstitution guide covers each step, and the units page explains syringe markings.
Questions
How much water do I add to a 10 mg vial of survodutide?
There’s no single right amount: the water only sets the strength. With 1 mL of bacteriostatic water, a 10 mg vial holds 10 mg/mL. With 2 mL it’s 5 mg/mL; with 3 mL it’s 3.333 mg/mL. Less liquid makes each dose a smaller draw; more makes small doses easier to measure.
How many units is 3.6 mg of survodutide?
With 1 mL in a 10 mg vial, 3.6 mg is 36 units (0.36 mL) on a U-100 insulin syringe. With 2 mL, 72 units (0.72 mL); with 3 mL, 1.08 mL.
How many doses are in a 10 mg vial of survodutide?
About 2 doses of 3.6 mg, or 2 of 4 mg or 1 of 6 mg. Taken 1× per week at 3.6 mg, it lasts about 2.8 weeks.
How long does a mixed vial of survodutide keep?
No label exists. Usual practice: powder frozen or refrigerated; mixed vials at 2–8 °C, protected from light.
